Prior to your first appointment, we will need your medical information. Please click the buttons on the side of the page to fill out our encrypted form, which will be automatically sent to our office, or print out the forms and bring them with you to your appointment. If you have any accessibility needs, or questions about the form's content, please arrive 20 minutes early to your first appointment, and we would be happy to help you complete the forms in-person.
AFTER HOURS EMERGENCIES
If you are a patient of Dentistry at the Inn on Lake Bonavista and you have a dental emergency, please call the office at 403-271-0106 to retrieve the emergency number.
